Talk. Learn. Connect. – 5 Wishes & Advance Directives

Talk. Learn. Connect (TLC) series provides an opportunity for those living with ALS, family members, caregivers, and friends to come together to share stories and hear the experiences of others living with the disease. While support groups are regional, anyone can attend any group that suits their schedule.

We will review the Five Wishes Living Will document. Five Wishes is a national advanced directive that allows you to indicate your personal, emotional, and spiritual needs as well as your medical wishes. Five Wishes is a living will that allows you to say how you want to be treated when you are seriously ill and at the end of your life.
